Glucose monitoring company Dexcom has partnered with digital chronic disease management platform Health2Sync to develop connected diabetes care offerings in Australia and Singapore.
The companies said the partnership will focus on helping people living with diabetes and healthcare providers access more connected approaches to managing the condition.
Health2Sync operates a digital chronic disease management platform used by around 1.7 million people across multiple Asian markets.
The platform is designed to help people with diabetes and their healthcare teams manage long-term care using digital health technology, while Dexcom provides continuous glucose monitoring technology.
The companies said the partnership reflects growing demand for technologies that support care coordination, simplify clinical workflows and enable more personalised management of chronic conditions.
